Pair Team is on a mission to connect underserved communities to high quality care as a tech-enabled medical group focused on high-risk Medicaid and Duals patients. Their novel whole-person care model - social, clinical, behavioral - is quarterbacked by a multidisciplinary care team across their network of shelters, food pantries, and other community partners leveraging Arc, their proprietary care management platform. Pair Team is one of the largest providers of a high-needs case management benefit in California and on track to build the country's largest clinically-integrated network of community-based organizations. They have demonstrated an ability to engage a hard to reach patient population and address gaps in care, including a 56% reduction in ED visits and a 26% reduction in hospital admissions.
As a Medical Director at Pair Team, you will play a critical role in supervising and supporting our team of Nurse Practitioners (NPs), owning the development and implementation of clinical policies and protocols, and collaborating closely with our Population Health team to ensure excellent outcomes for underserved communities. You will report to the Chief Medical Officer (CMO) and contribute to the strategic direction of the companys clinical operations.
This is a fully remote position with opportunities for in-person engagement. The role requires travel to represent Pair Team at conferences and with other partners and stakeholders. This role is externally titled Associate Medical Director (Value-Based Care) to accurately reflect its focus and scope. Internally, it is referred to as Medical Director.
Pair Team is an Equal Opportunity Employer. At Pair Team, we value diversity and strive to provide an inclusive environment for all applicants and employees. All applicants will be considered without regard to race, color, religion, sex (including pregnancy, gender identity, and sexual orientation), national origin, marital status, age, disability, political affiliation, military service, genetic information, or any other characteristic covered by federal, state, or local law.
