About Us

Hanna Center’s new Coordinated Specialty Care Early Psychosis program is a multidisciplinary, recovery-focused mental health clinic dedicated to early intervention for young people at risk of developing, or who have already experienced, a first episode of psychosis. Our program serves individuals ages 12-25, providing comprehensive care to reduce the risk of developing a psychotic disorder or supporting recovery following a first episode.

Job Description

Summary/Objective
The Psychiatric Mental Health Nurse Practitioner (PMHNP) will be responsible for the on-site delivery of psychiatric services to young individuals who meet criteria for Clinical High Risk for Psychosis (CHRp) or who have experienced a First Episode of Psychosis (FEP). This role involves close collaboration with the supervising psychiatric medical director, therapists, case manager, employment and education specialist, peer support specialist, and family support specialist.

Principles Duties and Responsibilities

  • Conduct psychiatric evaluations and ongoing assessments of CHRp and FEP patients.
  • Review outside medical records, perform diagnostic assessments including taking medical histories and performing physical exams, determine appropriate medical work ups including labs/imaging, communicate recommendations for diagnostic assessments to outside medical providers, interpret medical work up results, refer to outside medical specialists if appropriate, prescribe medication if appropriate, and monitor progress and treatment efficacy.
  • Perform crisis interventions, including but not limited to safety assessments and determination of 5150 appropriateness and execution.
  • Administer injectable psychotropic medications.
  • Establish and maintain therapeutic rapport with patients and productive working relationships with involved support persons.
  • Maintain caseload of 40+ patients.
  • Conduct frequent appointments as deemed appropriate to meet individual patient needs.
  • Maintain clinical records in accordance with mandated standards.
  • Collaborate with a multidisciplinary team that includes the psychiatric medical director, therapists, case manager, employment and education specialist, peer support specialist, and family support specialist.
  • Attend and participate in weekly staff meetings.
  • Attend and participate in clinical supervision with supervising psychiatrist.
  • Consult and provide ongoing education to non-medical staff in psychopharmacology and medical aspects of patient care.
  • Attend and participate in all assigned trainings for professional, program, and agency development.
  • Comply with all federal regulations for the sake of privacy and security of patient information.
  • Maintain quality assurance standards, adherence to MediCal/MediCare documentation requirements, and data collection for measured outcomes.
  • Additional duties, as needed, within the scope of practice.

Minimum Requirements:

  • Licensed to practice as a Psychiatric Mental Health Nurse Practitioner in the state of California.
  • Holds Nurse Practitioner furnishing.
  • 5+ years experience as a PMHNP.
  • 2+ years experience as a PMHNP working with early psychosis patients preferred.
  • Proficiency in psychiatric practice, including but not limited to patient safety best practices, psychopharmacology, general medical knowledge, and respect for patient autonomy and privacy.
  • Values collaboration and collegiality, and actively contributes to a positive team culture
  • Brings cultural awareness and humility to clinical work, with a thoughtful understanding of how individuals’ backgrounds, values, and lived experiences shape their care needs and interactions.
  • Basic Life Support certification.
  • Bilingual in Spanish preferred.
  • Effective communication skills to receive and disseminate information clearly and professionally.
  • Knowledge of basic technology, including Outlook and electronic health record systems.

  • Maintains accurate, timely documentation in adherence with MediCal/MediCare and HIPAA standards.
  • Utilizes electronic health records and digital communication tools efficiently.
  • Actively participates in training, supervision, and ongoing learning to maintain clinical excellence.

Comments

  • Must clear background check for a Community Care Licensed facility, including physical, TB, and drug test.
  • Sit for extended periods of time and the ability to manage repetitive hand/wrist movements while using a computer keyboard and/or the phone. Bend, squat, stoop, and walk- up a flight of stairs.
  • Lift and/or carry up to 20 lbs., occasionally. Duties are primarily performed in an office environment at a desk or computer terminal.

Work Environment
Typical office and clinical environment. Responsibilities may include offsite meetings, trainings, or other activities as required by business needs.
