The CNA/HHA will provide routine individualized healthcare based on a care plan set by the LPN to the elderly, convalescents, or persons with disabilities at the patient's home or in a care facility. Monitor or report changes in health status. May also provide personal care such as bathing, dressing, and grooming of patient. We are currently looking to fill part-time and full-time positions.ESSENTIAL DUTIES AND RESPONSIBILITIESThe essential functions include, but are not limited to the following:Provide companionship and socializationEscort clients to recreational activities and medical appointmentsPerform light housekeeping duties, as needed, to maintain a safe environment (i.e. changing bed linens, laundry, dishes, etc.)Help clients with personal care needs (i.e. Home duties, cooking, light cleaning, medication reminders, etc.)Assist clients with Activities of Daily Living (ADLs)Perform light housekeeping duties, as needed, to maintain a safe environment (i.e. changing bed linens, laundry, dishes, etc.)Teach and encourage self-care skillsMaintain complete and accurate documentation of clients' careMaintain confidentiality in all matters related to the client's carePerform other duties as assignedMINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)One year of Caregiving experience (preferred)Part-Time and Full-Time roles available, HHA (preferred) or CNA (required)ConnecticutDriver's License, Car Insurance, Car RegistrationStrong clinical assessment, organizational, and communication skillsAbility to handle multiple assignments simultaneouslyExcellent oral and written communication skillsExcellent customer relationship skillsPHYSICAL DEMANDS AND WORK ENVIRONMENT The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
