RequirementsResponsibilities/Day in the Life: Receive new patient referrals from clients Create charts for all new patients Input medical information in new chart Help Care Coordinators/Intake Coordinators with patient needs as applicable Foster relationships with their team to ensure the CareHarmony culture is positive and supportive. May be responsible for completing telephonic health assessments or chronic care management (CCM) calls with patients Other duties as applies Required Skills/Abilities: Great attention to detail Excellent organizational skills Excellent verbal and communication skills Excellent professional presence when dealing with colleagues Excellent time management skills with a proven ability to meet critical deadlines.
