Geisinger Wyoming Valley Hospital is a fully accredited Level 1 Trauma Program. The Trauma Case Manager plays an essential role in the operations of Trauma and Geisinger's strategic priorities. Responsibilities include daily review of clinical management guidelines for each patient, direct patient interaction, tracking quality performance indicators for accreditation, and rounding with the trauma clinical team. Duties include ensuring optimal quality, clinical and financial outcomes; networking and coordinating patient and family services; identifying and resolving negative quality, clinical and financial variances; creating action plans to optimize outcomes; acting as clinical expert; coordinating patient care including rehabilitation and social service consults; facilitating weekly multidisciplinary team meetings; participating in direct patient care as a preceptor; acting as preceptor or adjunct faculty for student nurses; participating in discharge planning; documenting care management and progress reports; providing consultation to multidisciplinary team members; assisting in data collection for Trauma PI Program and Trauma Registry; maintaining trauma roster; summarizing chart rounding information into quality improvement reports; coordinating data collection for audits; developing quality improvement instruments; and providing education on patient care and system. Work is performed in a clinical environment with accountability for job obligations and compliance with policies. Minimum 3 years RN experience required; Trauma/Critical Care experience preferred. Education required: Graduate from Specialty Training Program; Bachelor's Degree in Nursing preferred. Licensed Registered Nurse in Pennsylvania required.
