LanceSoft is seeking a travel nurse RN Case Management for a travel nursing job in Mission Hills, California.
Seeking an experienced Case Manager RN to provide care coordination and discharge planning within an acute hospital setting. Responsibilities include assessing patients, developing treatment plans, coordinating care across interdisciplinary teams, and ensuring regulatory compliance. The role involves utilization review, application of InterQual criteria, discharge planning, arranging post-acute services (SNF, DME, ambulance transport), and facilitating family meetings. Candidates must be comfortable working with diverse populations, including patients with mental health history, homelessness, or substance use challenges. Certifications RN license (CA), BLS preferred
