About The Position
Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty. License: Must have a valid nursing license. Certifications: CPR and BLS required. Must-Have: Proficiency in HomeCare HomeBase and Oasis, with use within the last 6 months. Description: Provides, coordinates, and directs the provision of home nursing care according to physician's orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. The role entails home visits, and participation in periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. Each type of visit and activity holds a different point value, totaling 30 points per week. Contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points. Types of cases include Start of Care, Wound Care, and follow-up visits. Candidates must have recent specialty experience; first-timers will not be accepted. Weekend requirements vary by location and profession. On-call requirements may vary; consult during the interview. Travel may be required up to 75 miles from the home clinic. Mileage will be reimbursed at the standard IRS rate. Local candidates are accepted. Pending licenses are not accepted; candidates must have a license in hand. Candidates should confirm their understanding of the nature of the home health assignment. Candidates must accept all patients scheduled, regardless of distance. All documentation must be completed within 24 hours of each appointment. There are no guaranteed hours with this contract. Clinicians must meet weekly productivity points. Availability to float to another nearby clinic may be required if needed.
