Job Overview

We are seeking a dedicated and skilled Step-Down Registered Nurse to join our healthcare team in providing exceptional patient care during the transitional phase from intensive care to less acute settings. The ideal candidate will possess comprehensive clinical expertise, including experience with complex medical conditions, advanced procedures, and diverse patient populations. The successful applicant will be instrumental in ensuring seamless care coordination, accurate documentation, and adherence to healthcare standards.

Responsibilities

• Monitor and assess patients' vital signs, symptoms, and overall condition to ensure stability during their transition from acute care settings.
• Administer medications and treatments with precision, including IV infusions, injections, and wound care such as suturing.
• Collaborate with multidisciplinary teams to develop and implement individualized care plans aligned with patient needs.
• Utilize electronic health record (EHR) systems such as Epic or Cerner for accurate documentation of assessments, interventions, and progress notes.
• Perform specialized procedures including catheterization, tube feeding management, and ventilator support when necessary.
• Conduct comprehensive patient assessments focusing on physiology knowledge, pain management, and functional status.
• Educate patients and families about post-discharge care plans, medication regimens, and lifestyle modifications.
• Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information.
• Participate in discharge planning by coordinating services such as home health or long-term care facilities.
• Engage in ongoing professional development related to trauma medicine, infectious disease care, geriatrics, or other specialized fields relevant to the role.

Skills

• Proven experience in step-down units or medical-surgical wards with exposure to critical care environments.
• Proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth for accurate medical documentation.
• Strong knowledge of ICD coding (ICD-9/ICD-10) for diagnostic evaluation and billing purposes.
• Skilled in advanced nursing procedures such as sterile processing, ostomy care, or neurovascular assessments.
• Familiarity with complex medical treatments including botulinum toxin therapy or hospice & palliative medicine practices.
• Ability to perform physical examinations and diagnostic evaluations confidently.
• Excellent communication skills for effective triage, patient education, and interdisciplinary collaboration.
• Experience working with diverse populations including pediatrics, geriatrics, patients with developmental disabilities or memory care needs.
• Knowledge of infection control protocols and aseptic techniques critical for safe practice.
• Capable of managing acute pain cases and providing comprehensive case management services.
