SUMMARY OF POSITION :

Part Time Positions are available for mobile wound care which is a 100% traveling position.

Compensation: $60.00 per hour. Opportunities for bonuses

Locations : Work in and around Fort Myers area with opportunities to travel farther when needed (possibly a 100-mile radius to serve our patients in outskirt communities).

Our overarching focus is to pair highly effective medical advances with the most patient populations suffering needlessly.

The position you are being offered is as an advanced registered nurse practitioner at our advanced wound care program will primarily be mobile visits. Applicants must be able to self-direct and motivate during assigned hours of service. Applicants must be able to drive to sites of service. (Financial stipends provided).

Position Description: The Wound Care Nurse Practitioner works with Heal Precisely’s Clinical and Office Team to provide wound care to patients in their homes, both in the community and independent living, as well as long term care rehab facilities. This position is responsible for providing wound care management in collaboration with the Medical Director and clinical team. The Practitioner is responsible for individualized plans of care for each patient resulting in improvements in ultimate wound outcomes. The Nurse Practitioner will do mobile visits with patients, provide telehealth visits as needed, and focused goals of full body approach to healing including, but not limited to, nutrition, medication, living conditions and best wound care practices available.

Training Provided & Supervision Received: Clinical Manager; Medical Director; Physician Steering Committee; Wound Care Experts & Medical Oversight and Support Department.

Supervision Exercised : Clinic based & Mobile care team.

Education/Qualifications:

  • Graduate of an approved Nurse Practitioner program
  • Certified by the American Academy of Nurse Practitioners and/or American Nurses Credentialing Center
  • Licensed by the Florida State Board of Nursing and in good standing with DEA license
  • BLS and CPR certified
  • Ability to react effectively in emergency situations
  • Ability to communicate clearly and establish/maintain working relationships with patients, medical staff and referral sources
  • Ability to work independently with interdisciplinary collaboration
  • Ability to drive from home to patient locations, as required
  • Maintain positive, motivated attitude

Physical/Mental Demands:

  • Requires full range of body motion to order to provide the care needed to patients in their environment
  • Requires daily travel
  • Frequent standing, walking for extended periods of time
  • Requires visual and hearing acuity
  • Ability to lift a minimum of 35 pounds and provide excellence in care despite varying community conditions

Essential Functions:

  • Develop and manage an individualized wound care treatment plan for optimal healing outcomes
  • Collaborates with facility team on wound management practices
  • Knowledge of up-to-date wound care products and therapies, including application and management of these products
  • Provides hands on wound care to patients including, but not limited to debridement of wounds by enzymatic or sharp methods when appropriate
  • Provides advanced wound care treatments based on patient needs
  • Orders, reviews, interprets and integrates diagnostic studies and laboratory findings for initiating or modifying the treatment plan
  • Prescribes medications and/or durable medical equipment to assist in healing of wounds
  • Provides the required billing information for services and maintains the documentation necessary for those services billed
  • Reviews patient prior medical records and maintains their current medical record related to the wound diagnosis and contributing co-morbidities
  • Patient chart encounters will be closed according to the standard of care
  • Identification of wound care quality measures, and provide data and documentation for tracking of quality measures
  • Instructs patient and family regarding wound treatments and management of plan for optimal wound outcomes
  • Participates in professional development activities
  • Completes required number of continuing medical education hours for license and certification
  • Attends required meetings, as requested
