Job Summary

We’re looking for a Massachusetts-licensed Nurse Practitioner to serve as the clinical linchpin of our intake process. You’ll combine the autonomy of independent practice with the satisfaction of knowing every decision you make directly unlocks support for a family in need. Your focus: Review, verify, and sign off on intake assessments conducted by our clinical team, ensuring every new member’s needs are captured accurately and that their enrollment moves forward without delay. You’ll connect with members via telehealth and in-home visits, close any gaps in their assessments, and act as a trusted clinical voice in our onboarding process. You’ll have the time, tools, and support to do the job right, without EMR red tape or a 20-patient-per-day grind

Key Responsibilities:

• Review and attest to comprehensive intake assessments completed by our nursing team.

• Conduct telehealth and in-home evaluations as needed to confirm or supplement assessment findings.

• Identify and escalate red flags – medical or social – to ensure members get timely interventions.

• Collaborate closely with intake nurses, care coordinators, and operations staff to accelerate the enrollment timeline.

• Document thoroughly, keeping all records compliant with state and payer standards.

• Serve as a resource for the team on clinical questions related to intake findings.

• Travel locally in Massachusetts for member visits; reliable vehicle required.

Qualifications:

• Current Massachusetts APRN license with Nurse Practitioner designation.

• Active national NP certification (e.g., ANCC or AANP) in Family, Adult-Gerontology, or related population focus.

• At least 2 years of NP experience (post-licensure) required; must be eligible for independent practice in Massachusetts.

• Current DEA registration and Massachusetts Controlled Substance Registration (or ability to obtain).

• Valid driver’s license, reliable vehicle, and willingness to travel for in-home visits across the state. (Mileage and gas for work-related travel reimbursed)

• Strong telehealth skills and comfort working in both remote and field settings.

• Meticulous attention to detail in documentation, with a bias for speed and accuracy.

• Comfort working in a fast-moving, high-accountability environment where “good enough” isn’t good enough.
